Oops, what exactly is estrogen? You can read everything you need to know here.

What is estrogen dominance?

The term estrogen dominance describes a hormonal imbalance in which there is too much estrogen in the body relative to progesterone. The estrogen does not necessarily have to be higher than the target value (direct estrogen dominance) – even a low progesterone level can lead to indirect estrogen dominance.

Estrogen is a central sex hormone that is involved in a multitude of metabolic processes. Therefore, estrogen dominance does not manifest uniformly: Every woman can develop completely different symptoms, depending on her hormone status, age, lifestyle, and genetic predispositions.

Symptoms of estrogen dominance

The more and more pronounced the symptoms occur, the more likely a hormonal imbalance is. In some women, many symptoms occur at the same time, in others only individual ones. Typical complaints are:

  • Heavy and prolonged menstrual bleeding

  • Pain during menstruation (dysmenorrhea)

  • Pronounced premenstrual syndrome (PMS)

  • Water retention, swollen fingers or legs

  • Breast tenderness or pain, especially 1–2 weeks before menstruation

  • Cysts in the ovaries or breasts

  • Headaches or migraines before menstruation

  • Mood swings, irritability, depressive moods

  • Myomas in the uterus

  • Fat deposits on the abdomen, hips, and buttocks

  • Hair loss

  • Fatigue, exhaustion, sleep disorders

  • Gallbladder diseases, especially gallstones

These symptoms can be exacerbated or triggered by estrogen, as the hormone affects the metabolism, nervous system, and immune function.

Causes of estrogen dominance

There are numerous triggers for an imbalance between estrogen and progesterone. The most common causes include:

  • Progesterone deficiency, e.g. due to chronic stress

  • Overweight, as fat tissue produces estrogen

  • Xenoestrogens and endocrine disruptors from plastics, cosmetics, cleaning products, etc.

  • Diet: high consumption of meat and dairy products

  • High liver burden from medications, alcohol, environmental toxins (slows estrogen breakdown)

  • Alcohol consumption, as alcohol can lower progesterone levels

  • The birth control pill or other hormonal contraceptives

  • The beginning of menopause (perimenopause), during which progesterone decreases first

  • Hypothyroidism

How can estrogen dominance be detected?

Observing the symptoms can provide initial clues. If you regularly notice several of the signs mentioned above, estrogen dominance may be present.

However, only a hormone analysis, ideally via a saliva test, provides certainty. Why saliva?

  • Only here are the freely available, active hormones measured
  • In contrast, blood tests often only show the total amount of bound (inactive) hormones
  • Hormones are very stable in saliva and the saliva samples can be stored well
  • can be obtained painlessly by yourself

Important: When determining the hormones, which have a natural rhythm, the timing of the determination from the saliva is very important. Therefore, always pay attention to the instructions. You should therefore know your cycle and its phases well.

Which values are important?

  • Estradiol (E2)

  • Progesterone

  • Progesterone / Estradiol ratio (P/E2)
The ratio should be at least 100:1 to rule out dominance.

Such a test can be ordered online from specialized laboratories and performed at home – or you can discuss it with your gynecologist.

Risk Factors and Amplifiers

Certain lifestyle factors or habits additionally favor estrogen dominance:

  • Chronic stress → Cortisol is produced from progesterone when needed in large amounts

  • Lack of exercise → Slows down metabolism and liver detoxification

  • Nutrient deficiency, especially magnesium, vitamin B6, zinc

  • Dysbiosis in the gut → affects the so-called oestrobolome, which is involved in estrogen excretion

A holistic view of nutrition, stress management, exercise, and gut health is therefore crucial.

Conclusion: Balance is the key

Estrogen dominance can have many faces – from migraines to myomas. The symptoms are individual and often diffuse. This makes it all the more important to listen to the signals of the body and gain clarity through a saliva test if necessary.

Those who know and prevent risk factors can actively support their hormone balance – completely without medication. Stress reduction, nutrition, physical activity, and a hormone-friendly lifestyle are often the most effective first steps.
